Kansas City Parks & Recreation proudly emphasizes sportsmanship in all athletic activities. Parents, coaches, and players each play a vital role in creating a positive, respectful, and inclusive environment both on and off the field.

Make an impact on the lives of youth athletes by volunteering to be a head coach! Youth coach volunteers are provided coaching resources, practice lesson plans and concussion awareness training to ensure they’re equipped with the tools necessary to foster a successful environment for all involved!

Purpose

Kansas City Parks & Recreation is committed to providing a safe, positive, and enriching environment for all participants. This application is required for all paid and volunteer youth sports coaches. Completion of this form and successful completion of a background check are required prior to participation.

Background & Eligibility Questions

Please answer YES or NO to the following questions. A “YES” response does not automatically disqualify an applicant but will require additional review.

1. Have you ever been convicted of, pleaded guilty to, or pleaded no contest to a felony?
2. Have you ever been convicted of, pleaded guilty to, or pleaded no contest to a misdemeanor involving violence, abuse, neglect, drugs, theft, or a crime against a child?
3. Have you ever been accused of, investigated for, or disciplined for inappropriate conduct involving a minor?
4. Have you ever been dismissed, suspended, or asked to resign from a youth-serving organization?
Code of Conduct Acknowledgement

  • Act in a manner consistent with the mission and values of Kansas City Parks & Recreation
  • Place the safety and well-being of participants above all else
  • Refrain from abusive, discriminatory, or inappropriate behavior
  • Follow all Parks & Recreation policies, including Inclement Weather, Heat, and Lightning protocols
  • Immediately report any safety concerns or suspected abuse
Background Check Authorization & Consent

I hereby authorize Kansas City Parks & Recreation to conduct a background check, including criminal history records, sex offender registry checks, and any other legally permissible records necessary to determine my suitability to serve as a paid or volunteer youth sports coach.

I understand that:

  • Information obtained will be kept confidential and used solely for program safety and employment/volunteer eligibility purposes
  • Falsification, omission, or misrepresentation of information may result in denial, dismissal, or removal from coaching duties
  • Background checks may be repeated periodically during my service

I release Kansas City Parks & Recreation, its employees, agents, and background check providers from liability arising from the lawful collection and use of this information, except where prohibited by law.
